Requirements for Application:
  • Must complete a career day through Fox Valley Healthcare Alliance before April or have completed a career day in the past
  • Interested in a health career
  • Currently a member of the junior class
  • Agree to uphold ethical and confidentiality standards
  • Attend kick-off meeting with a parent or guardian
  • Have required immunizations and health record up to date
  • Commit to 12 hours within specific week assigned
  • Submit completed application online and turn in two or three references by Mar 2nd
Currently enrolled in or registered for the following courses:
  • Chemistry
  • Anatomy and Physiology
  • Advanced Biology
  • Health Related Careers
Requirements once Accepted:
  • Complete Blood Borne Pathogens Training
  • Turn in a copy of your immunization record
  • Attend the meeting, turn in Confidentiality Agreement with signatures of both you and a parent
  • Complete and turn in Evaluation form after completion of your shadowing experience
