All students who are interested in job shadowing at the Orthopedic and Sports Institute need to read and fill out all of the required forms. Print off the health requirements and confidentiality agreement forms, fill them out and bring them with you the day of your shadowing. Read the Bloodborne Pathogens training manual and take the test online. Print off the Evaluation form and bring to fill out after you day of shadowing. (See menu on the right.)
